I have been a dentist for 22 years. I graduated from the University of Pennsylvania School of Dental Medicine. I have had my own practice for 16+ years. It has been successful by any measure. Except one. My happiness.
So I have decided to change the way I run my clinic. I have cut my schedule in half. And as a result my income has been cut in half as well. My clinic is fee for service. I will not allow insurance companies to dictate or influence my care for my patients based on money. I will no longer tolerate their attempts to demonize me or my integrity so that they can justify their less than honorable tactics solely designed to save money while deflecting blame.
I will continue to file claims for my patients and make every attempt to maximize their benefits to get the most out of their dental plans. I will continue as a provider for Selected Administrative Services (SAS). Their fee schedule is very close to my own. And they rarely use the tactics of denying, downgrading, bundling and other tactics used by most insurance carriers to keep from paying for treatment.
I will spend more time with my patients. I will continue my conservative approach to dental care. With the primary focus being to keep and repair their natural teeth for as long as possible.
I will no longer do amalgam (silver) fillings. They are ugly and they are not better structurally than resin (tooth color) fillings. I am looking for two things from my patients.
First, I want patients who care enough about their oral health that they show up for their appointments. It's also the only way I can pull off reducing my schedule.
Second, once concerns and questions about proposed treatment have been satisfied I expect to move forward with that treatment. If you are not this patient you will likely be better served going to a dentist who is too busy to worry about your oral health. i want a partnership with my patients.
I have lost patients who didn't think I had the time to fully engage them about their oral health. And I have lost patients because we were too busy to ensure their treatment was handled efficiently and or timely. Those are my biggest regrets. I would love to go back and repair those relationships. Instead I'll have to settle for doing the right thing now.
